Winter Sundays at the Main Library

The Farmington Main Library is open Sundays, 1:00 PM - 4:00 PM, January 7, 21, 28, February 4, 11, 25 & March 3, 10, 17, 24.

Good Friday Closing

The library will be closed Friday, March 29 in observance of Good Friday.

Passports & Proctoring

PASSPORTS, NOTARY & PROCTORING

The Farmington Library is a passport acceptance facility. Applications will be accepted by APPOINTMENT ONLY. Contact Information Staff at 860-673-6791 for information regarding evening and weekend appointment availability; Monday - Thursday and some Saturdays.  Due to high demand appointments may not be immediately available, and book one to two months in advance.

We ARE NOT taking photos at the Library at this time. You can get photos made at CVS, the UPS Store, AAA and Walgreens.

Getting A Passport

Make an appointment by giving us a call at 860-673-6791

  • Both parents/guardians must be present with photo IDs
  • If one parent cannot be present, a notarized DS-3053 form and a photocopy (front and back) of the absent parent’s ID is required (Notarized with 90 days)
  • If minor only has one parent/guardian; a court order of sole custody, death certificate or judicial declaration of incompetence is required.
  • Completed DS-11 form
  • Evidence of U.S citizenship (such as long-form birth certificate with raised seal and both parents names or naturalization paper, find out more here)
  • Old passport if renewal; lost passport required DS-64 form
  • Passport photo
  • 2 checks – U.S. State Department and Farmington Library

  • Applicants between the ages of 16 and 17 years require parental awareness (signature or parent’s check) if they cannot provide a driver’s license
  • Out-of-state applicants must provide secondary ID

Application is submitted by mail by you! You do not need to make an appointment.

Passport Fees

Requires 2 checks – one payable to the U.S. State Department and one payable to the Farmington Library

Adults – $130 to the U.S. State Department  + $35 to the Farmington Library

Minors – $100 to the U.S. State Department + $35 to the Farmington Library

Proctoring

Proctoring for exams is available at the Main Library. Exams will be completed in a quiet study room. The quiet study rooms do not have computers. Staff can hold the student’s phone and other items at the Information Services desk while they are completing the exam.  Information Services staff members are unavailable to sit in the room with the student during their exams.

The library can mail back completed exams (student pay for mailing) or scan and email exams back to the school.  There is no charge for exam proctoring beyond postage, if necessary.
